About this PreSchool

Enrollment:

Applications are accepted year-round from families who meet the eligibility criteria.

The Head Start program runs throughout the school year;

Early Head Start runs year round.

If classes are full, they will add your child's name to a waiting list.

To start the enrollment process, simply contact Early Head Start at (616) 453-4145, ext. 2288 or the Head Start location nearest you and schedule an appointment.

During the appointment, a staff member will help you fill out the necessary paperwork.

Please bring the following information with you:

• Proof of income (last year s W-2, CDC, TANF, SSI, pay stubs)
• Proof of birth date for each child to be enrolled.
• Current immunization records
• Recent physical (within one year)
• Dental examination report (within 6 months)
• Medicaid
• Insurance card
