ENROLLMENT REQUIREMENTS

1. Copy of Birth Certificate
2. Immunization Records

3. Income documentation for the last 12 months which can include:

1. W2, Tax Return
2. SSI Documentation
3. TANF Documentation
4. Child Support Printout
5. Work One Wage Inquiry
6. Pay Stubs
7. Scholarship/Grants/Student Loan Documentation
8. Foster Care StatementIf interested or need more information
9. fill out their online Inquiry Form.
