About this PreSchool

To apply for Head Start or Early Head Start, please call 211 or 739-4211 to arrange an application appointment.

A biological parent or a guardian must complete the application.

For the application appointment, you must bring:

Income Verification for the last 12 months (i.e. tax return, W2, FIP printout, check stubs, foster care printout, pay envelopes or written statement from employer)

To complete the application:

• Current Copy (within the past year) of child's physical
• Current Certificate of Immunization, signed by a medical professionalIf applicable
• Copy of child's IEP or IFSPNames and phone numbers of child s dentist, doctor, and additional contactsBirth Certificate is not required
