About this PreSchool

To be considered for Head Start or Early Head Start, you will need to complete an application.

They will also need to see the following:

If you are employed, one of the following:

• A check stub
• Your W-2 form
• A letter on letterhead from your employer stating the days, hours, and wages per hour that you work
• Proof of any other form of income, such asTANF letter from DSS (if applicable)
• SSI letter from the Department of Social Services (if applicable)

Verification of birth:

• Birth certificate, orBirth letter from the hospital
