Criteria for enrollment information is neededApplication must meet income guidelines as established by the Department of Health and Human ServiceAge Eligibility

Applicants must be three (3) years of age as of September 1st.

The following information is needed at the time of appointment

• Birth Certificates of all family members
• Social Security numbers of all family members
• Check Stubs
• 1040
• W2 forms
• Utility bills
• Proof of residency
• Immunization card for applicant
