OfficeParents must bring the following documentation for enrollment:

Income Verification for household for one month (for example: Tax Forms, W-2 Form,
Unemployment compensation, Pay stubs)
Medical insurance card
Proof of address
Updated physical exam (within one year)
Birth Certificate
Social Security Card
