About this PreSchool

They are always looking for children to enroll in their program.

Please follow these instructions:

1. This is a fill-in form.

Type your information in the allowed fields.

Certain fields are meant to be left blank and will be filled in by their staff.

2. Print out the form.

3. Call the site nearest to your home and make an appointment for the next step in the registration process.

4. Be sure to bring the form with you to the appointment along with any other information you are advised to bring.
