Criteria for enrollment information is neededApplication must meet income guidelines as established by the Department of Health and Human ServiceAge Eligibility -

Applicants must be three (3) years of age as of September 1st.

The following information is needed at the time of appointment

• Birth Certificates of all family members
• Social Security numbers of all family members
• Check Stubs
• 1040 and W2 forms
• Utility bills
• Proof of residencyImmunization card for applicant
